Eligibility & Making Changes

Active, full-time employees working at least 30 hours or more per week and their dependents are eligible for benefits on the first day of the month following your date of hire.

Eligible dependents include:

  • Your legal spouse or domestic partner
  • Your natural, adopted, or step-children, up to age 26
  • Children of any age, if incapable of self-support due to mental or physical disability

Making Changes

The choices you make when you first become eligible are in effect for the remainder of the plan year. Once you are enrolled, you must wait until the next Open Enrollment period to change your benefits or add/remove coverage for dependents, unless you have a qualified change in family status as defined by the IRS.

Examples include, but are not limited to, the following:

  • Marriage, divorce, or legal separation
  • Birth or adoption of a child
  • Death of a dependent
  • Loss or gain of other health coverage for you and/or dependents
  • Change in your work status that affects your benefits
  • Change in eligibility for Medicaid or Medicare for you or a dependent
  • Receipt of a Qualified Medical Child Support Order

Keep in mind that you may be required to provide supporting documentation, such as a birth certificate or marriage certificate, to prove your dependent’s eligibility. Contact Human Resources within 30 days from the status change or within 60 days if the status change is due to Medicaid/CHIP events.

Note: You have 30 days from the life event date to make changes to your coverage and provide supporting documentation to prove eligibility. Email benefits@cradlepoint.com to get started.
