Life and Accidental Death and Dismemberment (AD&D) insurance, through Unum, provide financial security to you and your family if you become seriously injured or pass away.
Providing economic security for your family if you pass away, become disabled, or experience an injury or illness is a major consideration in personal financial planning. As an employee, you automatically receive Basic Life and AD&D coverage in the amount of $100,000. This coverage is provided at no cost to you.
You may choose anyone to be the beneficiary of your Life and AD&D policy in the event of your death or serious injury. Review your beneficiary designation periodically to ensure it reflects your current wishes. You may change your beneficiary as often as you need in ADP.
Voluntary Life and AD&D offers additional coverage at discounted rates. The chart describes the amounts of coverage you can buy for yourself, your spouse, and your child(ren).

*Evidence of Insurability (EOI) may be required. Newly eligible employees are not subject to EOI up to guaranteed issue amounts. During Open Enrollment, employees with Voluntary Life in place, may increase their voluntary life by 5 increments ($50,000) without EOI, not to exceed $250,000 for employee coverage, or $50,000 for spouse coverage.
Note: Spouse life insurance amount can only be up to 50% of the employee’s life insurance election.
Life can have its ups and downs. With Unum disability coverage, you can be protected financially if you experience an injury or illness that prevents you from working.
Effective 1/1/2022: STD coverage provides income replacement if you are sick or injured and need time off work to recover. The STD plan provides 100% of your weekly salary, to a maximum of $2,500 per week for up to 12 weeks of a disability. Benefits begin after a 7-day waiting period for illness or injury. If you are hospitalized, the waiting period is waived. You are automatically covered under the STD plan at no cost to you.
Disability benefits may be offset by benefits you receive from state-mandated disability plans.
LTD coverage is important because anyone at any age may become injured or ill for an extended period of time. LTD protects you from loss of income in the event you are unable to work due to an illness, injury, or accident for a period of time that exceeds the short-term disability benefit. You are automatically covered under the LTD plan at no cost to you.
LTD coverage will replace 60% of your base salary to a monthly maximum of $6,000 if you are disabled for more than 90 days and are unable to work. LTD benefits are offset by other sources of income, such as Social Security and workers’ compensation.

If you travel for business, you can have peace of mind knowing Cradlepoint offers Group Travel Accident insurance, through Zurich, to eligible employees. In the event of your death or dismemberment while on a business trip, your beneficiaries will receive $250,000. GTA coverage starts when you leave your home or worksite and ends when you return home or to work, whichever occurs last. Please note, your regular commute to and from work is not covered by this plan.
